Program Application
The following checklist and information will help you through the process
of applying for the nursing program.
1. Attend an information session
-
The purpose of the information session is to learn more about the
nursing program, the program admission standards and program
participation requirements.
-
For the schedule of information meetings, please check out
morainevalley.edu/nursing. Reservations for an information
session can be made by calling (708) 974-5355 between 8:30 a.m. and 5 p.m. Monday through Friday. When you check in for
the Information Session, you will need to present a government-issued
photo I.D. or a Moraine Valley photo I.D. Seats are limited for each
Information Session. Call today to reserve your seat.

Attendance must have been within a year from application to the program.
NOTE: At the Information Session, you will receive your "Authorization
to test" card to take the ATI TEAS test as well as the Nursing Program
application.
2. Submit the Moraine Valley enrollment form
-
If you have NOT previously applied to Moraine Valley, this is your next step.
-
You may enroll online
or in person at the Registration Office in the Student Services Center,
Building S.
3. Complete placement and admission tests
-

ATI Test of Essential Academic Skills (TEAS):
1. You must have completed a Moraine Valley enrollment form to be able to take
this test

Prior to taking TEAS the applicant must:

Attend an information session.
-
Receive a referral card for the TEAS test at the end of information
session.

Submit the Moraine Valley college enrollment form.
-
Make an appointment with the Testing Center by coming into B-101 or
calling (708) 974-5309 to take TEAS.
Day of TEAS testing:
-
Candidate pays $40 testing fee payable to Moraine Valley Community
College by check, money order, cash, credit or debit cards.
-
Candidate presents a cashier stamped paid form for TEAS testing fee.
-
Candidate presents referral card.
-
Candidate presents government-issued photo I.D. or Moraine Valley photo
I.D.
-
Candidate is assigned to computer, signs in, and is registered to take
the exam.
-
The computer is reserved for this specific candidate for 209 minutes (3
hours and 29 minutes).
-
Upon completion of exam, the candidate receives a copy of tests results.
-
Candidate is referred to the Academic Advising Center to review test results and
their implications.
Retesting with TEAS
-
Candidate has the option to test twice on the TEAS, there are currently
two different forms of the exam.
-
First testing is scheduled by the student for any testing date.
-
Second testing, MUST have a 10-business-day waiting period from the
first testing and it is recommended that there be some type of
remediation intervention before the second testing.

Candidate makes an appointment with the Testing Center for second
testing date.

Pays $40 to Moraine Valley Community College
-

4.
Submit a Nursing Application with:
A. Acceptable Test Scores
1. COMPASS Mathematics and Composition/Reading Requirements:
-
Mathematics: Based on Compass placement test results or ACT
tests, or previous college credit with a grade of "C" or better, all
within two years of application deadline, applicants must be qualified
to take MTH-098 or equivalent.

Composition/Reading: Based on COMPASS placement test results, or
ACT tests, or previous college credit with a grade of "C" or better, all
within five years of application deadline, applicants must:

ATI TEAS Requirements:
-
Submit your nursing application with acceptable ATI TEAS scores
to the Registration Office, Building S.
-
Acceptable scores for EACH of the following test areas is 60 percentile.
-
The test cannot be taken more than two
times in an application cycle. There must be 10 business days
(Monday through Friday) between each test attempt. Remediation
between test taking attempts is encouraged.

If you have taken the ATI TEAS test
elsewhere you can use that test results if they meet the acceptable
test scores and are within one year of application deadline. To have
these results shared with Moraine Valley, you can
-
contact ATI and request the official
test transcript be sent to Moraine Valley Records Office, or
-
you can sign a release of information
for Moraine Valley to make the request of ATI for your test transcript.
-
Your Moraine Valley enrollment form must already be on file.
B. Transcripts and Proof of current CNA registry to the Registrar's
Office (If transcripts were mailed to the Records Department, it is YOUR
responsibility to confirm that they were received and are on file).

Official high school transcript: The current transcript must show
ether the graduation date of ongoing student status. The GED reports
must show passing scores.
-
Official college transcripts: Transcripts from other colleges
must be submitted if you think you have completed courses that may
fulfill admission requirements or support course work required for the
nursing program.

Illinois CNA registry: Proof of CURRENT registry status must be
provided. This information can be accessed at
www.idph.state.il.us/nar/home.htm. Print a copy of your registry
form from this Web site and submit it with your materials.
Ranking and selection
1. Ranking score:
a. TEAS score in Math and Science (combined raw score)
b. If you have completed College BIO 180 Anatomy and Physiology prior to
application: point(s) will be awarded based on the final grade: C = 1
point, B= 2 points, A =3 points.
2. Residency: Proof of Residency must be submitted with your
application. You must provide two of the following with your application:

Copy of this year's income tax return with the W-2 attached
-
Copy/proof of automobile insurance card
-
Copy of most recent real estate tax bill
-
Copy of current signed lease with name and phone number of the landlord
-
Copy of current voter's identification card
These
items must be in your name, not a parent or other person. We understand that
this may be an inconvenience, but we wish to remain fair and equitable in
our handling of selective admissions programs.
Permanent residents who submit all required documentation to the
Registration Office, Building S, by the application deadline will be admitted in
ranking score order before out-of-district residents.
3. Tie breaker: only if necessary date/time of
completed NURSING application
4. Applicants will be notified of the status of their
selection within three weeks after program application deadline.
5. NOTE: Applicants who decline a seat within 14 calendar days of the
beginning of the semester or once the semester begins will not be able to
reapply for the nursing program in the next admission cycle.
Academic
Standing
-
BIO-180 Anatomy and Physiology must be completed with a "C" or better
PRIOR to admission to any NUR courses.
-
Any general education course that applies to the nursing program
completed prior to enrollment in the program must be completed with a
grade of "C" or better.

As per the Moraine Valley policy course work can only be repeated twice without
